Answer: Arizona Revised Statutes, section 25-903 sets forth the grounds for dissolving a covenant marriage. Adultery and living apart for two consecutive years are both grounds for dissolution of a covenant marriage under the statute. The self help center on the Maricopa County Superior Court's website has forms that can be downloaded and filed with the Court. The Dissolution forms on the website are for a non-covenant marriage. Your best bet is to hire a lawyer to draft custom forms for you, but you may be able to modify the self help form on the website to suit your needs.
