Question: My Birth Certificate reads Kitrina and I have been writing as Katrina since high school. Everything I own is in Katrina, Driver's Lic, Soc Sec card, Bank Account, Home ownership. With the new Driver's Lic requirements my Birth Cert. would not make it possible any longer to have it Katrina they will only issue with Kitrina. Do I have to do a legal name change?
Answer: The Vital Records Section of the Arizona Health Department can help you with this. You are applying to correct a misspelling of your name. Go to: https://www.azdhs.gov/licensing/vital-records/index.php#corrections-amendments-birth-certificate. The site will give you information and telephone numbers for the vital records department. They can arrange an appointment for you to talk to someone and get the process started.
