Question: How do I change my children's last name to their biological father's last name?
Answer: You can find detailed information and instruction on how to change your childrens' names on the Arizona Department of Health Services, Vital Records Bureau. adhs.gov/vitalrecords. Those instructions will tell you that if your children are more than one year old, you will need a court order for the name change. The web site will tell you what documents you will need to submit to the court. You can also call and schedule an appointment to meet with a vital statistics clerk who can walk you through the process. The telephone number in Maricopa County is (602) 364-2428.
