Question: Can I get a new birth certificate with my new name after a name change. I heard I can but I still want to make sure. Can that even be possible. What do I have to go through to get a new birth certificate with only my new name.
Answer: If you have a court order changing your name, you can then go to the Bureau of Vital Records and have your name changed on your birth certificate. The website for the Bureau of Vital Records is as follows: www.azdhs.gov/licensing/vital-records/index.php#marriage-divorce-records. You can also call and make an appointment at (602) 364-2428. The website contains a wealth of information on the documents you will need and the process.
