Question: I recently divorced, and due to an error the request to restore my maiden name was not checked on the decree. I currently live out of state for graduate school, though I still hold an AZ driver's license and remain registered as a resident in Pima County. Since I am out of state, how do I now go about getting my name restored?
Answer: Under A.R.S. §12-601 a party may file an application for a name change in the superior court of the county in which they reside. The application for Pima County Superior Court may be found on the court’s self-service center. A link has been provided here: http://www.sc.pima.gov/Portals/0/Library/civil/NameChange_Adult.pdf In Pima County the cost for filing a name change application is $244. The cost can be expensive, however, you may request a deferral or waiver of costs.
