Question: I have a 6 month old son ahd i want his last name changed to his real father but I hand my ex-boyfriend that lived with me, he wanted to be his dad so my son has his last name we thought it would work, but it didn't. Now the real father wants to give him his last name, how do I get my son's last name changed to his father's?

Answer: Amendments can be made to a birth certificate issued in Arizona, through the Office of Vital Records. If the birth certificate was issued in another state, the Arizona Office of Vital Records has a list of offices in other states. Depending on how much time has elapsed, a Court Order may be required to change the birth certificate.  The website for the Office of Vital Records suggests contacting them to verify what documents are needed.
