Forms to terminate guardianship are available at any of the Clerk of the Superior Court office locations; however each county may require additional forms. This Court Locator provides locations and websites for the Superior Courts of every Arizona county.
Please refer to the Superior Court which the request for termination of guardianship is to be filed in for all forms and filing instructions. This website outlines the process, and provides the forms, for terminating guardianship in Maricopa County.
