Question: I am about to give birth in a week and i was wondering if i am able to put my babys father last name even if hes not gonna be there when i give birth. We are not married and hes from mexico.

Answer: Getting the father's name on the birth certificate happens in one of two basic ways.  If the parents are married, dad's name is automatically included.  If the parents are not married, both the mother and the father must sign an affidavit of paternity.  If dad is not present when the baby is born, the father's name will be left blank.  However, you can go to Vital Records at the Arizona Department of Health Services and amend the birth certificate.  I am giving you a link to the Vital Records website that will give you more information on what you need to do.  He will be able to sign the affidavit in Mexico and send it to you for your signature and filing.  Go to: https://www.azdhs.gov/licensing/vital-records/index.php#corrections-amendments-amend-to-acknowledge-paternity.
