Question: I was born & raised in Phoenix, Az. When I was born my mom thought my dad sent in my paper for birth certificate and my dad thought my mom sent it in. In all reality it didn't get sent in at all, in the required time frame deadline so now my birth certificate reads (not named) and my last name with all of the other info re: my birth (parents names, etc) How do I get my first & middle name added to my birth certificate? I need to have this done for the new Real ID & so I can switch things over to a new state. ( driver lic, etc) can this be done from out of state & how much would it cost? Thanks

Answer: Go to https://www.azdhs.gov/licensing/vital-records/index.php#corrections-amendments-registrants-name.  That is the website for the Vital Records section of the Arizona Department of Health Services.  On the site, you will be able to read all the rules and requirements for amending your birth certificate to add you first name and middle name.  In Maricopa County, the fee to change your name on a birth certificate and get a certified copy is $30.00.  You can find the fees for each county on the website.  The website also has telephone numbers so you can call and talk with someone who can answer any questions you may have.
