Question: I was married at the time my two children were born, my ex husband left the state and haves nothing to do with my kids and I want to know how I can take his name off of the birth certificate and change my kids last name to my maiden name? Thank you
Answer: To change the birth certificates and last name, go to http://www.azcourts.gov/selfservicecenter/Self-Service-Forms#NameChange. You can take the information to the superior court nearest you to file for the changes. For birth certificates you may want to go to: http://www.azdhs.gov/licensing/vital-records/index.php#birth-certificates-who-can-obtain for information. You may also wish to contact an attorney for more information or advice.
