Question: The man listed on my AZ birth certificate is not my real father. Am I able to change this information? What would the process be?
Answer:
In order to amend one’s birth certificate, one must fill out a Petition for the superior court of Arizona. Below is a link providing the paperwork and instructions on filling out the paperwork needed to proceed with the change.
