Question: My ex husband was ordered by the court to provide me with his address and employment information within 10 days. It's been well over the 10 days and he refuses to give me the information. What forms do I file to request contempt charges and that he be forced to give me the information?
Answer: You may choose to file a "Verfied Petition for an Order to Appear" and an "Order to Appear". Forms and instructions can be found at either the Pima County Superior Court, Maricopa County Superior Court or Coconino County Law Library websites. They have "self help" sections and you can look through the family law forms. There is no precise form to address your issue but there should be a "Request to Enforce Support" form that you may be able to modify for your purposes.
