Question: If I am the respondent . what name goes in the person filing on the form myself the respondent, or my spouse the petitioner

Answer: If you, as the respondent, are filing a pleading in response to a petition filed by your spouse,  you list your name as the respondent and sign the document as the respondent.  In the caption section at the beginning of the document, you put the entire caption as it appears on the petition and the case number.  The caption identifies the petitioner and the respondent.
