If you are in Maricopa or Pima County, they have a self help service on line for downloading and filling out the forms. Go to: http://www.superiorcourt.maricopa.gov/ezCourtForms/index-kiosk.asp for the forms and instructions. You may also want to check with an attorney to see how to get child support stopped.
