Question: How to change the name of my Newborn Child? Is a hearing still required?
Answer: If you and the father are willing to sign an affidavit and agree on the name change, you should be able to change your baby's name through the Vital Records Department of the Arizona Department of Health Services. All of the information you will need is on the website. Go to https://www.azdhs.gov/licensing/vital-records/index.php#corrections-amendments-registrants-name. You will find a list of everything required based on age groups. If your child is a newborn as you state and less than 90 days old, the first set of criteria will apply.
