You will need to complete an application for certified copy of birth certificate, a copy of the front and back of your valid government-issued picture identification, along with a cashier's check or credit card information to AZDHS P.O. Box 2111 Phoenix, Arizona 85001. Per the AZDHS, you will need to include a self-addressed stamped envelope so the copy may be mailed to you.
