My father died in March & mother in June. We have found that dad had some life insurance through USPO with no beneficiary designated. They want to pay to my mom's estate but there is no estate. She had assets under $10,000 and no real property. I found the small estate affidavit. There are three surviving children, 1 deceased with 2 children. I assume we all need to fill out an affidavit? One of the grandchildren is still a minor. Who fills out his?

My ex- fiancée and I have joint custody(decision making). Last year March17, 2014, she was arrested for domestic violence( class 6 undesignated felony), ( discharging a firearm at her boyfriend),and received supervised probation, 3 yrs. In June of 2015, she was once again arrested for violation of probation and placed on IPS and extensive probation till 2017. Because Mother is violent and has potential to cause emotional and physical distress to my 5 yr. old daughter, do I have legal right as her father to file for sole custody and decision maker? Currently we have equal parenting time.

I've been calling several places and they wouldn't be able to do that because they are not doing the service free or their charge are too much. How do I determine my wife's legal name? Prior to marriage and according to her birth certificate, her name was [first name] [mother's maiden surname] [father's surname]. After marriage and consistent with her cultural practice, she dropped her mother's maiden surname as a middle name and now uses [first name] [Father's surname] [husbands surname]. There is no indication of the name change on the marriage certificate. How do we know what is the proper name to use for driver's licenses, passports etc.?

I have a question regarding medical insurance. When we were divorced 11 years ago my ex husband was ordered to carry the medical insurance on our son. He was fired from his job about a year ago and found something else that did not have medical insurance so he applied for a "discount" medical card. I have since put our son on my medical insurance to make sure that he has the best coverage. Is there any way to have him pay the premium?
