Answer: Finding Paperwork: Forms for obtaining appointment of guardianship and/or conservatorship can be downloaded at www.superiorcourt.maricopa.gov by clicking on "Where do I…Find the forms.” This link will connect you to a page titled “Self-Service Center” under the section “Getting forms and Instructions” and “Categories of Forms” click on “Probate.” Here you will forms for guardianship and/or conservatorship for both adults and minors. Filling out paperwork: Court staff cannot give legal advice or help you fill out forms. If you need legal advice or help filling out forms, you have several options: 1.) You can see if the Court offers a workshop on the specific form you need to fill out. 2.) On the “Self-Service Center” page you can click on the “Self-Help and Educational Videos” section to see if there is an educational video related to the topic for which you need assistance.
