I am scared for my children. What is the name of the packet in the self service center I can use to request this be done. The hair folicle test. He can trick the urine test. Thanks.

Answer:

There are self service centers at many different court locations around the state of Arizona. To find out exactly what forms are available, you may want to check with the court closest to you. You can check the court's website or visit the self service center in the court. Here are some links for a few of the court self service centers in Arizona:

Maricopa County

Pima County

Coconino County

Also, some of these locations may have a blank motion form in order for a person to request something of the court that falls outside of the specific court forms.
